Dorchester County Health Department

The primary mission of the Dorchester County Health Department (DCHD) Coordination of Community Services (CCS) Program is to assist people with developmental disabilities in leading fulfilling lives. Coordinators work with participants to identify their preferences and needs to assist them with developing a person-focused plan of supports. Coordinators focus on community-based supports and services which facilitate the health, safety, and personal satisfaction of the individuals. We believe that individuals with disabilities have the right to access community-based services and supports of their own choosing. As a program operating within the local health department, we are intertwined with all of the components offered within.  The CCS program has also established long standing community relationships through partnerships in county interagency councils.Service Area: Queen Annes, Talbot, Caroline, Dorchester, and Wicomico. We also serve Somerset and Worcester on a case by case basis.
