Caroline County Health Department

The DDP is currently a Medical Assistance Provider and is licensed as a Targeted Case Management Provider by DDA within the Maryland Department of Health. The mission of the Caroline County Health Department (CCHD) Developmental Disabilities Program (DDP) is to provide a high quality, intensive and individually based program of assessment, planning and monitoring for individuals eligible for funding through the Maryland Developmental Disabilities Administration (DDA). We believe that individuals with disabilities have the right to access community based services & supports of their own choosing.
