The Coordinating Center

Our mission is to partner with people of all ages and abilities and those who support them in the community to achieve their aspirations for independence, health and meaningful community life. When you work with The Coordinating Center you are encouraged, supported and enabled to meet your own personal goals. Services and supports are provided in an environment that promotes maximum independence, community inclusion and quality of life. We offer: assistance with transitions (e.g., school-based services to adult services, hospitals/institutions to homes in the community, pediatric to adult health care); coordination of affordable and accessible housing; Cup of Joe's for support and networking (3rd Friday of every month); and, we maintain contact with our participants and families often. We employ more than 200 coordinators who support more than 9,200 individuals enrolled in Medicaid programs and waivers.
